Video Loan Policy
R-Rated DVDs and videotapes: Loan Policy and Form
It is the policy of the Morton Grove Public Library that all DVDs and videotapes, regardless of MPAA rating, may be borrowed by any patron, regardless of age, who holds a valid Morton Grove Public Library card.
However, parents may request that their child not be allowed to borrow R-rated DVDs and videotapes by completing a DVD/Videotape Loan Form (below). Completed forms will be kept on file at the Check Out Desk and a corresponding notation will be placed on the child's record. Library staff will then permit the child to borrow only G, PG, PG-13 and unrated DVDs and videotapes.
In the case of a child for whom a DVD and Videotape Loan Form has been filed, the restriction will expire upon receipt of proof of the child's 18th birthday. It shall not be the responsibility of the Library to verify any child's age.

By filling out this form a parent may request that his or her child, under the age of 18, not be allowed to borrow R-rated DVDs and videotapes.

not be allowed to borrow DVDs and videotapes from the Morton Grove Public Library that have been rated "R" by the Motion Picture Association of America. I understand that my child may still borrow G, PG, PG-13 and unrated DVDs and videotapes.
Unless earlier withdrawn by me, this request expires upon receipt of proof of the child's 18th birthday.
