Up Molasses Mountain by Julie Baker, 2002 "When union members arrive
to organize their West Virginia coal mining town, fourteen-year-old
Clarence Henderson, shunned for his cleft lip, and his neighbor
Elizabeth Braxton narrate the changes in their own lives and in the
lives of everyone in their community."
Adult fiction:
Courting Trouble by Lisa Scottoline: "Centers on the all-woman
Philadelphia law firm of Rosato and Associates, led by Benedetta
(Bennie) Rosato. Scottoline's previous novels revolved around Rosato and
her two associates, but this one introduces a new character, Anne
Murphy, fresh out of law school. She's brainy, beautiful, and nervy, and
oh yes, she had a cleft palate as a child, doesn't speak to her mother,
and is being pursued cross-country by a stalker.... "
Red Dragon by Thomas Harris, and the movie based on it, Manhunter: The
character of Dollarhyde has scars of a procedure to fix a harelip and
cleft palate.

Not quite a "main" character, but one of the half-dozen or
so main characters: one of the regulars in Joe Gores' DKA
Associates crime/detective series is a repo man with a
cleft palate. He's hired about three books into the
series--I think in 32 PINK CADILLACS, but could be wrong.
And I can't recall his name.
Even less a main character: one of Penrod's kid associates
in Booth Tarkington's books (PENROD; PENROD AND SAM; PENROD
JASHBER) is a young African American boy, one of identical
twins named (sigh) "Herman and Vermin." They can be told
apart only because one has a harelip and the other does not.
