Thanks to the people who suggested answers, but I believe I've found the story in "Cinderella Outgrows the Glass Slipper and other Zany Fractured Fairy Tale Plays" by J. M. Wolf.
"Cinderella dreams not about going to the ball, but about going to school to become a veterinarian." Another twist is that the fairy godmother joins the tooth-fairy union.

The cover indicates the book is for grades 3-5, but it is rather amusing for all ages.
