Stumper: Contest, flying, thumbs-up!

Patron remembers this scene from a novel, which may be at the
beginning:
Small town woman wins a contest of some kind and wins a plane ride.
Problem is, she is terrified of planes, but feels pressured to go
through with it. The pilot tells her to signal if she wants the plane
to head back to land, and when in her nervousness she gives the pilot a
thumbs-up, he takes it as a "go higher" signal. As you can imagine, no
fun for the terrified woman.
Patron was thinking it might have been in a Kingsolver or Flagg novel.
Ring any bells for anyone out there?
