$40? I've never ordered any.

If the question is "why exactly $40," I don't know (and they do
have a few items at higher or lower prices).
If it's "why so expensive, in the $40 range," this is typical for
small press hc publishers in the sf/f field (and probably in
other fields as well). Small presses don't have the economies
of scale that big commercial presses do and generally concentrate
on doing hardcovers that are sturdy and attractive than major
publishers, but that appeal to a more limited (though faithful)
audience--sometimes a very limited one indeed. I don't know
about Cemetery Dance (I think I have a couple of their books
but no title comes to mind), but the small presses I'm addicted
to like Midnight House, Tartarus Press, Sarob, and especially
Ash-Tree Press generally do editions of from 250 to 500 copies
and charge $40 to $60 apiece for them.
Detailed bibliographic information on small press output in the
sf/f field for the last ten or so years can be found in the
online Supplements to the Mirage Press INDEX TO THE SCIENCE-
FANTASY PUBLISHERS, by the way. Original volume is available
only as an o.p. hardcover or an available cd-rom, but the
supplements can be viewed at:

where this press appears as "CD Publications."
In the latest supplement, the press immediately following them is
Charnel House, with one new title: a collection of song lyrics
with two hc editions: one at $200 and one at $600. Special case
and different marketing strategy, but still makes CD (or Ash-Tree)
look good by comparison....
