Could this be Anya Seton's Green Darkness?. I read this so long ago and
can't remember the ending but check out a sysnopsis on Amazon and see if
it fits. At any rate, this is a good story with some of the elements you
suggest and might be a good read while trying to find the book you speak
of.

An unlikely possibility or another title of interest: Enchanted by Virginia Henley features a woman time traveling back to Ancient Rome where she meets and falls for the Ancient Roman incarnation of a man she knows in her time (18th-ish century?). There is another man in the picture in the contemporary story.
And now for a related question...
There is another title involving past lives and couples who are meant to be together across time, but I cannot find the title or remember the author's name, and so I append my own stumper. It's a rather tragic story, told from the contemporary point of view of a couple who is having marital difficulties and having odd experiences. The story is ultimately that they were a doomed couple in a past life (he was a priest/monk and she was maybe supposed to marry another man), she got pregnant and they came to a bad end. They are maybe living in the same house in the present where they lived in the past. The events of the past life are mirrored in the contemporary life up to the point that the present people are "taken over" by their previous selves and try to act out the final violent hours of their past life. There was something about a closet/confessional where she hid or was attacked. The author's name may be something like Marston and I'm sure the title has the word "green" in it. I checked it out from the Williamsburg Regional Library in Virginia in 1996 or 1997, in case any WRL folks are here :)

Another good title along these lines is "The Dragon Rises" by Adrienne
Martine-Barnes. This follows the eternal love triangle through time - a
trio of spirits who act out the triangle again and again. For instance,
they were Arthur, Gweniviere and Lancelot in another life. This is
science fiction with fantasy elements, and a favorite of mine.
