ago. The story was read by Scott Simon and was probably a picture book. In it
a dog has lost his collar and cannot remember his name, so he asks all the
other dogs if they remember it. (I guess Simon did each dog voice with a
different accent). They don't remember his name but do remember little bits
about him. For example, "Oh, you're the dog that always loses his collar,"
etc. We have tried to find it on the NPR website, Amazon, OCLC, etc, but have
had no luck. Does anyone remember this book?

I suggest a couple of options. Scott Simon's show on NPR has a web site.
The URL is: <http://www.npr.org/programs/wesat/rundowns/> It lists the
archives of the show, and you keep going back deeper until you get to the
actual show. The other alternative is simply to email the show and ask if
they have a better way of tracking down the info about the book.
Found the URL using "Scott Simon" + NPR as the search key, on dogpile.com

Another thought - maybe the story in question was by Daniel Pinkwater, who is
often on NPR with Scott Simon.
