book ID -escape, river, mountains

I do remember the scene in question involving water. The woman and the
guide have gone to ground in a cave, but are being chased by the Bad Guys.
Due to a huge rainstorm, the underground river inside the deep cave cuts
them off from help and escape, and they somehow find their way through a
narrow passage in a harrowing scene where both are in teeth-clenching danger
and terrified to bits. I do think the woman has to go on without the man
because she's smaller and can fit through the narrow escape route.
As melodramatic as it sounds, it was a great read that kept me on
tenterhooks till the end.

Subject: ?help with book identification
I come to the collective wisdom to ask if anyone can offer any clues to
the identification of a book. I wasn't given a great deal of information to
go on....
Patron read the book a couple of years ago in hardcover. She only remembers
the following details:
There is a scene where a man (most likely the villain) and a woman who was
trying to escape from something come to a river while trying to cross a
mountain range. The only way to do this is to swim under the water and come
up in certain location where there is an opening to an escape route. The man
show the woman where to access this opening. There is a second crossing
scene where the man may not make it all the way. She reports that this scene
was very suspenseful and gripping and would like very much to read the book
again.
Any suggestions very much appreciated!
